If you discover damages to your packages after you’ve signed the delivery receipt, please contact us via the info issue form immediately. We also recommend filing a claim with the courier service provider.
Notating Damage
Please take pictures of the damaged products and notate any damages on the delivery receipt. Be as specific as possible, note each identified issue, and be sure to include a video recording of the package.
Damaged items must remain in their original packaging and/or damaged state until proof of damage and/or insurance representative has been established.
Filing Claims of Damage
Phrozen's courier partners are under contract to fulfill orders accurately, so if any shipment has been confirmed to become damaged during transit, customers can file a claim. We encourage you to provide not only required package information but also additional supporting documents. Supporting documents can include:
- Proof of Delivery.
- Proof of Lading.
- Photos of damaged packages and contents.
- Proof of Value documentation, including:
- Copy of original invoice from vendor/supplier.
- Final confirmation screen of an online order with proof of payment
- Appraisal(s) - Expense statement
